Pain relief can fall short after gynecologic surgery

NEW YORK (Reuters Health) - For some women having a hysterectomy or other gynecologic surgery, at-home plans for pain relief may not be good enough, according to a new study.

Of 127 women who had their surgeries at one Utah medical center, about half told researchers their pain control was falling short three days after leaving the hospital.

Two weeks later, 23 percent of women whose surgery had involved cutting into the abdomen were still feeling their pain wasn't controlled -- as were five percent of those who had vaginal surgery.

Even more were still in need of narcotic painkillers at the two-week mark: one-third of women who'd had abdominal surgery, and 10 percent of those who'd had vaginal surgery.

The study, reported in the journal Obstetrics & Gynecology, is a rare inquiry into how women fare during at-home recovery from gynecologic surgery.

?It's just amazingly understudied," said lead researcher Dr. Ingrid Nygaard, of the University of Utah School of Medicine in Salt Lake City.

One reason it's so important to understand what goes on during home recovery is that hospital stays are now much shorter than they were years ago.

The women in this study were discharged from the hospital an average of one or two days after surgery, depending on whether it was vaginal or abdominal. Most often, the surgery was a hysterectomy (removal of the uterus) or a procedure to correct prolapse -- when weakened tissue in the pelvis allows one or more organs to protrude into the vagina.

All of the women kept diaries at home to record their pain control after surgery.

Based on the results, Nygaard's team concluded pain relief was ?suboptimal" for some women.

?I'm not advocating more narcotics," Nygaard said, referring to the powerful painkillers, like hydrocodone and oxycodone, that surgery patients typically need for a period of time after their procedures.

But, she said, there's a need for more research into how to best control women's pain after gynecologic surgery.

?We can see that it's a problem. Now we need to see what helps," Nygaard said.

For now, better patient education on pain control might make a difference, she said. Some women, for example, might find that it's best to take non-narcotic painkillers -- namely, non-steroidal anti-inflammatory drugs like ibuprofen -- ?round-the-clock," and save narcotics for more-occasional use.

Attention to relieving constipation could help too, Nygaard said.

Many patients have constipation after surgery, and narcotics tend to make the problem worse. Having smaller meals, eating more fiber or getting light exercise might help get the bowels moving -- and ease any pain from the constipation, Nygaard said.

In theory, ?complementary" remedies like massage or gentle stretching exercises could also help, she noted. But there's still a need for studies to show whether that's actually the case.

How do you know if your discomfort after surgery is ?normal" or not?

According to Nygaard, one ?red flag" is if you're still in need of narcotics after two weeks. But each patient has to talk with her own doctor about the best way to manage her pain.

One ?striking" finding from the study, Nygaard noted, is that about half of the women said they still did not feel generally ?recovered" six weeks after surgery.

So if you're still not feeling yourself at that point in recovery, you're not alone. ?It's not abnormal to not feel fully recovered at six weeks," Nygaard said.

LONDON (AP) - Manchester United surged closer to a 20th English league title on Sunday, opening an eight-point lead by beating Queens Park Rangers 2-0 while floundering neighbor Manchester City lost 1-0 at Arsenal.

Having spent most of the season in City's shadow in the Premier League, United has eclipsed its big-spending rival to take control of the Premier League title race.

"We know what it takes to get over the finishing line first," United defender Rio Ferdinand said. "We've got to use all that experience to make sure we do it again."

United has six games remaining to extend its title record after its eighth successive league win.

Wayne Rooney scored the opener from the penalty spot after QPR captain Shaun Derry was sent off in the 15th minute by referee Lee Mason for pushing over Ashley Young, although there was minimal contact and the United winger appeared to be offside.

Cruise Shipping Miami 2012 Closes with ... - Travel World News

Organizer?s for the 2012 Cruise Shipping Miami convention and exhibition announced record-setting attendance and participation with a 6 percent rise in exhibitors and?4 percent more attendees from 120 countries at its 28th edition. The global exhibition and conference attracted almost 11,000 attendees between Monday and Thursday,?March 12-15.

?I?ve spoken with a number of exhibitors who were very pleased with the trade show traffic they experienced this year,? said Daniel Read, director of UBM?s Cruise?Shipping Group. ?Indeed, 51 percent of this year?s exhibitors have already confirmed for the 2013 show.?

The annual conference and trade show started by remembering the victims of January?s Costa Concordia accident at the opening State of the Industry session. However, a?recurring theme of the cruise industry?s resilience emerged during the opening plenary session and continued through panel discussions on such topics as the next hot?global source markets, new destinations, opportunities in Europe and Asia, and the annual U.S. Coast Guard forum.

Among the significant news made during CSM 2012 were:

Carnival Cruise Lines announced the 1996-built Carnival Destiny will receive a $155 million refurbishment at Italy?s Fincantieri shipyards, emerging from dry dock in April 2013 with a new name ? Carnival Sunshine.

MSC announced it intends to launch another ship in 2013, bringing its fleet to 13 ships. MSC Preziosa will be the fourth ship in the Fantasia-class, along with MSC Divinia, which launches in May.

Norwegian Cruise Line unveiled new details about Norwegian Breakaway, which will enter service in April 2013. The ship offers 17 dining venues and 12 different bars and lounges, including an outdoor area known as ?The Waterfront.?

Princess Cruises also offered more details on dining options on the upcoming 3,600-passenger Royal Princess, an entirely new class of vessel for the line that debuts in 2013.

New to the four-day conference in 2012 were two panels on emerging medical services and technologies at sea, an update on the burgeoning China cruise market and a panel on port and cruise ship safety and surveillance. An expanded U.S. Coast Guard forum covered a wide range of issues, from regulation compliance to examinations and safety and security.

The 29th annual Cruise Shipping Miami Convention is scheduled for March 11-14, 2013, at the Miami Beach Convention Center.

There were lots of little character moments in this episode.? First off, why was Kate sleeping on Lauren's sofa? I'm sure there's at least one guest room in that home.? And if Kate's been there for weeks why hasn't she bought some food rather than griping about what's in Lauren's fridge.?

I know Kate works long hours but she could take an hour and do some grocery shopping, or how about looking for a place of her own.? It's not like she can't afford it so what's holding her back?

Despite her progress, Kate still has her issues like when she yelled at Justin in the middle of his fundraising dinner. I know she was upset about Danny but making a scene during something that was important to Justin seemed childish.

Lauren's control issues definitely came to the forefront as she tried to reign Ben in but I must admit that Ben did come off kind of sleazy for most of that case.

The implication of his sexcapades with Lydia at Alcatraz really didn't help.? If you've never been, Alcatraz is great to tour but it's old, dirty, and dangerous.? Not a place to consider taking off articles of clothing but maybe that's just me.

But it was good to see Ben take control of the case and then remind Lauren that he's also a partner, not a child for her to look after.? Of course he did a pretty good imitation of a mischievous brat so I couldn't blame Lauren entirely.

My favorite moment of the show was the last conversation between Kate and Justin.? In this Fairly Legal quote, Justin tells her?

During that moment I hated that they were now divorced.? Their chemistry is one of the highlights of the show.

California Workers' Compensation Looms as a Major 2012 ...

On March 28, two California legislative committees met to hear concerns about the California workers? compensation system.?The chairs of the committees declared that the hearing was the Legislature?s first-step in this year?s effort to solve problems that plague the system.

During the joint hearing of the Assembly Insurance Committee and the Senate Labor & Industrial Relations Committee, stakeholders in the California workers? compensation system identified problems and gave their perspectives on how those problems should be addressed.

Representatives of the California Workers? Compensation Institute outlined the increase in workers? compensation costs.?In the years immediately after the enactment of the 2003 and 2004 reform laws, the total loss per indemnity claim decreased.?However, in recent years, workers? compensation claim costs have been increasing.?The total loss for an indemnity claim is higher today than prior to the enactment of the 2003-2004 reforms.?Institute data show that escalating medical costs are driving the increase in claim costs.?Increasing costs are affecting insurers. The most current accident year combined loss and expense ratios are at 130.

Insurance Commissioner Dave Jones observed that the high combined ratios will probably result in a rise in workers? compensation insurance rates.?The commissioner expressed concern about the higher premiums that may be charged to employers.?In wrestling with workers?? compensation issues, the Legislature has operated under the theory that a dollar increase in benefits should be accompanied by a dollar in savings in the workers? compensation system.?Commissioner Jones explained that because of the sharp increase in costs, that theory is no longer useful.?It appears that it will take more than one dollar in savings to offset a dollar in benefit increase.

Christine Baker, director of the Department of Industrial Relations, testified that her department is seeking comprehensive workers? compensation reforms that achieve both cost savings and benefit increases.?Baker explained that such comprehensive reforms will require both legislative and regulatory changes.?The Division of Workers? Compensation is conducting public forums throughout the state aimed at reaching a consensus on the changes that should be made.

Frank Neuhauser, professor at the University of California at Berkeley argued that the 2003-2004 reforms have reduced compensation paid to injured workers. Neuhauser said the reforms resulted in a 61% decrease in overall compensation.?He stated that workers who are not represented by attorneys have been especially affected by the decline in compensation paid.

A representative of the California Federation of Labor accused insurers of undermining the workers? compensation administrative process and delaying medical treatment for injured workers.?The Federation called for the prior approval of workers? compensation insurance rates and significant adjustments to the permanent disability rating schedule.

A representative of Grimway Farms, which is self-insured for workers? compensation, challenged the allegation that high costs can be solved by stricter insurance regulation.?As a self-insurer, Grimway is facing the same increase in workers? compensation costs as insurers.?The Grimway representative complained that there are too many lawyers in the workers? compensation system. ?A representative of public schools urged the adoption of measures to reduce the number of workers? compensation liens.

A representative of the California Medical Association asserted that further restrictions on fees that may be charged for workers? compensation medical treatment would lead to a reduction in access to care.?A representative of the California Society of Industrial Medicine and Surgery complained about delays in utilization reviews and the administration of medical provider networks.

At the close of the hearing, Senator Ted Lieu, chair of the Senate Labor & Industrial Relations Committee, and Assembly Member Jose Solorio, chair of the Assembly Insurance Committee, said that they are committed to achieving both workers? compensation savings and workers? compensation benefit increases.?The committee chairs said that they will proceed in an honest, cautious and transparent manner.
